Arena Information, Concussion Protocol & Consent to Treat Forms

As we start our season we’d like to highlight and share some good to knows.    
Consent To Treat/Medical History Form - Waiver Form (click here for copy of form)

It is mandatory that every player bring a completed/signed copy of the Consent To Treat/Medical History Form to their first ice session and turn it into their coach.

Concussion Protocol
To help ensure the health and safety of the skaters please know that it is protocol that our coaches and officials have the authority to pull any player from the ice if they suspect a concussion. If a coach/official removes a player from the ice due to a suspected concussion, it's the parents' responsibility to have the player examined by a medical professional.  The player will need a note signed by a physician to return to the ice.
