Referees Incident Report

Incident Reports and when not to use them.

DO NOT USE AN INCIDENT REPORT FOR GAMES RELATED MATTERS.

Incident reports are used to report behavior witnessed by an official that occurs OFF THE ICE by non-participants of the game. Fans ejected for Zero Tolerance violations, fans fighting in the stands, officials confronted and/or verbally abused off the ice surface by anyone. Threats made to an Official leaving the building is an example of things reported on the Incident Report.

This report is to be used to inform the District Supervisor of Officials of any incidents that occur at a Minnesota Hockey game, scrimmage, or tournament that involves spectators, rink personnel, players, coaches, minor officials, team officials, or any other people who are not covered in the Playing Rules. Such incidents might involve verbal or physical abuse of an official, an altercation between spectators, confrontations in a parking lot or anything else that is detrimental to the game of amateur hockey and displays poor sportsmanship. Please fill in the information requested below, and send this report to your District Supervisor of any incidents that occur at a Minnesota Hockey game, scrimmage, or tournament. If you don't know the details, just relate what you saw.
